Aim 11: Initiate use of SSRI’s with children with diagnosed depression and / or Anxiety disorders.

Aim 10: Screen all adolescents for depression (e.g. PHQ-9).

Aim 9: Screen all adolescents for suicidality (e.g. ASQ).

Aim 8: Administer standardized youth- and/or parent-report rating forms to assure that all initial depression diagnostic evaluations include objective information (e.g. PHQ-9, Columbia Depression Scale)

Aim 7: Measure depression symptoms using a standardized parent and / or youth form at each visit to monitor symptom improvement.

Aim 6: Measure anxiety symptoms using a standardized parent and / or youth form at each follow-up visit to monitor symptom improvement.

Aim 5: Obtain standardized ADHD parent and / or teacher-report rating forms at each scheduled follow-up visit for youth with ADHD to monitor symptom improvement.

Aim 4: When starting an ADHD medication, schedule a follow-up office visit within a 30-day period of the first prescription appointment to assess symptom response and possible side effects.

Aim 3: Administer standardized youth- and/or parent-report rating forms (e.g. SCARED) to assure that all initial anxiety diagnostic evaluations include objective information from caregivers and / or youth.

Aim 2: Administer standardized parent- and / or teacher-report rating forms to assure that all initial ADHD diagnositic evaluations include objective information from caregivers and / or teachers.
